Has anyone hacked together a menu addition for Ruby? This would include
stuff like commenting, uncommenting, running the buffer and stuff like that?

I don’t know about running the buffer, but the standard M-x
comment-region and uncomment-region work for me in the ruby major mode.
It’s not in the menu, but you can bind a key sequence to it.
